Ah allow me to demonstrate:

In your usual seating position, grab the phone as you would when browsing the net or reading an email.  Now, and this is the trick, turn it upside down while keep holding it at your normal reading level. You'll notice the cam is on now the bottom side of the phone. Adjust slightly if needed, press click et voila!  discreet as it can be
(provided it is on silent)
