Rear naked choke?

I have oft jokingly referenced it as a shaving scar and/or knife fight wound, but tis neither. I don't want to meet the cat that can apply a rear naked that does this type of damage.

Hint 2: It's actually two scars, a scar within a scar.

I have a couple more hints I  can drop before I spill the beans. Unless otherwise requested, I will try to make revelation in a timely manner, likely 24 hours. I really am interested to see how others take to the thread and share, the guesses alone have made me chuckle.

Was it a wound that was cauterized? The scar within a scar thing…

No, each scar was applied over a year apart.

Hint 5: the incision sight was not where the majority of my symptoms occurred. To explain better I would need a hand.
