The Medium is a 38 (that is, it is designed for someone with a 38 chest).  The size chart measures the actual chest measurement of the jacket, which in this case is 40.6.  G

Jz714, I'd say go for a Large: it will give you that extra length (+2cm) and width at the bottom hem (+4cm) that you need, the sleeves might then be a bit too long, but after a good deal of wear they will shorten a bit due to the forming of the arm combs…

For comparison: I have a 40" chest and wear just about every IH shirt in size L, but for this jacket I went XL (tagged 42), ...

yeh after few hours of wear, its killing my shoulders, its just a tad too small, and i was only wearing a skin tight T…

Now i have to sell/exchange it...damn
